SoonerSelect Healthy Behaviors Program

It’s rewarding to visit your dentist! Liberty’s Healthy Behaviors Program helps our members take charge of their dental health. As a member of the SoonerSelect Dental Program, you or your child may be eligible to receive a reward.

If you are covered under the SoonerSelect Dental Program, with Liberty and are

• A child under 21, or

• Pregnant, and

• Have not seen a dentist as a Liberty member in the last 12 months.

You may be able to receive a reward by joining Liberty's Healthy Behaviors Program and completing a dental checkup or preventive care service.

Joining the Healthy Behaviors Program is simple!

1. Fill out our Oral Health Risk Assessment or call Member Services.

2. See a dentist for a dental checkup or other preventive care service.

3. A reward will be mailed or emailed to you.*

For help enrolling in the Healthy Behaviors Program, call us toll free at 888-700-1093 (TDD/TTY: 877-855-8039) and let the agent know that you or your child would like to join the Healthy Behaviors Program. You can speak to a live person Monday through Friday, between 8 a.m. and 5 p.m. PT.

*Final rewards are subject to program eligibility and all the conditions mentioned above are met. We base our data on dental claims and other sources. Please allow up to 6 weeks to receive your reward. Eligible for a reward once every 12 months. While supplies last.
