Liberty Dental Plan’s Healthy Behaviors Program helps our members take charge of their dental health. As a member of the Nevada Medicaid Program, you or your child may be eligible to receive a reward.
Who is eligible?
If you are covered under Nevada Medicaid, with Liberty and are:
An Infant under age 1, or
Between the ages of 1 – 20, or
An adult 21 or older enrolled as a pregnant member, and
Have not seen a dentist as a Liberty member in the last 12 months.
You may be able to receive a reward by joining Liberty’s Healthy Behaviors Program and completing a dental checkup or preventive care service.
How does it work?
Joining the Healthy Behaviors Program is simple!
Get a dental checkup or other preventive care service by requesting an appointment using the link below.
Opt in to Liberty’s text messaging program and provide a valid cell phone and email address.
Done! A reward will be mailed or emailed to you.1

For help requesting an in-person visit, call us toll free at 866-609-0418 and let the agent know that you or your child would like to join the Healthy Behaviors Program. You can speak to a live person Monday through Friday between 8 a.m. and 5 p.m. PT (TDD/TTY users call 877-855-8039) to help you schedule your next dental visit.
1Final rewards are subject to program eligibility and all the conditions mentioned above are met. We base our data on dental claims and other sources. Please allow up to 6 weeks to receive your reward. Eligible for a reward once every 12 months. While supplies last.
