Liberty's Healthy Behaviors Program helps Medi-Cal take charge of their dental health. You may be eligible to receive a reward.
Who is eligible?
If you are covered under Medi-Cal, with Liberty, and have not seen a dentist as a Liberty member in the last 12 months, you may be able to receive a reward by joining Liberty’s Healthy Behaviors Program and completing a dental checkup or preventive care service in person or virtually.
How does it work?
Joining the Healthy Behaviors Program is simple!
1. Get a dental checkup or other preventive care service by requesting an appointment using the link below.
2. Opt in to Liberty’s text messaging program, and provide a valid cell phone and email address.
3. A reward will be mailed or emailed to you.*

Call us toll free at 888-703-6999 (TDD/TTY: 877-855-8039) and let the agent know that you or your child would like to join the Healthy Behaviors Program. You can speak to a live person Monday through Friday, between 8 a.m. and 5 p.m. PT to help you schedule your next dental visit.
*Final rewards are subject to program eligibility and all the conditions mentioned above are met. We base our data on dental claims and other sources. Please allow up to six weeks to receive your reward. Eligible for a reward once a calendar year. While supplies last.
