Welcome to the Kaiser Permanente Smile Diabetes Program. We specifically designed this program for our dental plan members as a resource to improve your oral health and overall well-being. We understand the important link between oral care and diabetes management and our program is tailored provide our members with additional educational tools and support. If you choose to enroll, you will have access to this program at no cost.

Eligibility criteria:
You are an active member of Liberty Dental Plan.
You are an active member of Kaiser Permanente Health Plan.
You are 18 years or older.
You have been diagnosed with Type 2 diabetes.
