Stopping and correcting fraud, waste, and abuse (FWA) in healthcare is essential to Liberty's success.
Liberty requires all our employees and all staff in our provider network to complete FWA training annually.
Examples of fraud, waste, and abuse
Changing an invoice or receipt to secure payment.
Billing for a more expensive service than provided.
Submitting a claim for services not provided.
Falsifying a patient's diagnosis to justify an unnecessary procedure.
Misrepresenting a procedure to secure payment for a non-covered procedure.
Billing multiple times for the same service.
Using another person's identification or insurance card without authorization.
